1. Your Minecraft name:
2. Your Twitter handle:
Note: If you're not using the same forum account that you had as an admin & your Minecraft name and Twitter handle are not the same as when you we're a past admin, you will have to verify your identity with another admin that can vouch for your identity.
3. What rank were you (super, telnet, senior) and the approx dates you were an admin: (e.g. super admin from Jun 2010 - Jan 2013)
4. Were you removed from admin for misconduct or reasons other than inactivity? If yes, give full explanation.
Note: Admins who are removed for misconduct or other reasons besides short term inactivity must file 3 valid griefer reports during their period of suspension in order to apply for reinstatement. If this applies to you, how many griefer reports have you filed? Those who have been inactive for greater than 4 months may have to fulfill this requirement at the owner direction.
5. Why do you wish to be reinstated?
6. If you are reinstated, how often will you be able to do admin duties?
7. Are you completely familiar with the server rules and banning protocols? A Senior or Telnet admin will be testing you on this before you get admin restored.
Admin Instructions: Except for when an admin was removed for misconduct, applications for reinstatement will normally be automatically approved and either the owner or executive admin are the approval authority. For cases of reinstatement following a removal for past conduct, the approval authority is the owner and will normally be approved provided the applicant has cleared up their past misconduct issues. Admins do not normally need to vouch or object but can if they wish, esp if they have current information on the applicant that would be helpful in making a decision. Unlike initial applications for super admin and promotion applications, former admins who are loyal to the server, and desire to serve as admins and overcome past mistakes will normally be given the benefit of the doubt and provided extra chances to succeed or fail.
